Editorial and corrections policy

People use these pages to know what they will pay for a medical service. An old deductible or a wrong rate can make them put off care or overpay, so every figure points back to the official file it comes from.

Who is responsible

Radif Partners, contact@outofpocket65.com.

What we read

  • the quarterly CMS fee schedules: physicians (RVU file), hospital outpatient (Addendum B), surgery centers (Addendum AA), medical equipment (DMEPOS) and ambulances;
  • the yearly CMS fact sheet with the Part A and Part B deductibles, premiums and copayments;
  • the medicare.gov coverage pages, one per service, and the Medigap benefits chart;
  • the Code of Federal Regulations (42 CFR) when a payment rule is set there.

Sources we refuse

Insurers, agents, plan comparison sites, blogs and other websites never provide a figure here. A rule we cannot find on an official page is not published.

Review cycle

The medical equipment and ambulance fee schedules are downloaded again automatically at the start of each quarter, from the site's launch. The physician, hospital outpatient and surgery center schedules are updated by hand: the published version is the October 2026 one, and the next manual update is planned for January 2027. The yearly amounts are corrected by hand when the November fact sheet comes out. Every page shows the date of its last review.

Corrections

  1. We open the official file or page again.
  2. We correct the data and add a test for the case.
  3. The pages rebuild with the new date.

Independence

No insurer, agent, hospital or agency pays for or reviews what is published here, and the site carries no affiliate link. See also the method page. Version: 2026-10-24.
